Background: Iron deficiency anemia is the most common
micronutrient deficiency worldwide, characterized mainly by abnormally low
hematocrit and hemoglobin levels. The treatment starts with managing the
underlying causes and replenishing the iron store by oral iron therapy. Iron
supplements usually associated with side effects that decrease patients’
compliance. Patients may fail to respond to oral iron supplements and be
refractory to treatment due to multiple factors. Clinical pharmacist's role in overcoming
these factors and improving iron deficiency anemia management should be
investigated.
Objective: Our goal was to investigate the role of
clinical pharmacist on health outcomes of iron deficiency anemia patients in
North Jordan and to investigate the effect of iron deficiency anemia on
patient's quality of life at baseline and after follow up.
Methodology: Patients who attended Princess Basma
Teaching Hospital Outpatients Clinics and were diagnosed with iron deficiency
anemia were invited to participate in the study. Patients who agreed to
participate were randomized into intervention or control groups. Patients in
the intervention group were offered pharmaceutical care services including
patient counseling, and the clinical pharmacist worked closely with their
physicians to manage their iron deficiency anemia. Patients in the control
group received usual medical care. Patients in both groups were followed for 4
– 6 weeks.
Results: One hundred and four patients were
enrolled in the study. Eighty two patients completed the 4-6 weeks duration,
with 43 patients in the intervention group and 39 in the control group. By the
end of the study, 86 % of intervention group patients and 59 % of control group
patients reached their hemoglobin goal values with statistically significant
improvement in intervention group versus control group (p value = 0.006).
Moreover, health related quality of life assessment showed significant
improvement in the intervention group compared with control group patients
mainly in pain, movement and depression domains. Approximately, 84 % of the clinical
pharmacist recommendations were taken by physicians.
Conclusion: Clinical pharmacist implementation has improved iron deficiency
anemia management outcomes and also health related quality of life.
